What should I to bring to my appointment?
- Please complete the New Patient Packet of forms prior to your initial exam, which includes Patient Registration, HIPAA, Consent for Treatment and Financial Agreement, and MRI Procedure Screening Form for Patients.
- Any cards or operative reports pertaining to implants
- Insurance card
- Driver’s License
